Italy

Healthcare rights of migrants

Asylum seekers:  Asylum seekers and beneficiaries of international protection are required to register with the National Health Service and enjoy equal treatment with Italian citizens through insurance provided by the NHS. This includes those out of reception. Medical assistance is extended to each regularly resident family member under the applicant’s care in Italy and is recognised for newborn babies of parents registered with the National Health System. The right to medical assistance should not expire in the process of the renewal of the permit of stay. However, asylum seekers with an expired permit of stay have no guarantee of access to non-urgent sanitary treatments. 

Refugees: Are entitled to free healthcare and can enjoy the same benefits as nationals.

Undocumented migrants: Can apply to a local national health service office for a temporary (six-month) health card. Through this health card they are entitled to urgent care, essential care, preventive care including maternity care and the diagnosis and treatment of infectious diseases.

HIV testing and treatment

Testing: Free in certain places. Private laboratories charge a fee and certain hospitals may require co-payments. Tests are anonymous. Use the European test finder to see where you can get tested for HIV. 

Treatment:  Free for all, although treatment is low and not available for those in prisons.de who and when to treat. Undocumented people may sometimes be excluded.

  • LHIVE Diritti e Prevenzione

    LHIVE Diritti e prevenzione, formerly the Italian League for the Fight against AIDS (LILA) – Catania, was established on 27th of May 1991, on the initaitive of some healthcare professionals. LHIVE Diritti e prevenzione promotes and defends the right to health, organizes campaigns to reduce the spread of sexually transmitted infections, works to affirm principles and relationships of solidarity, fight against all forms of violation of human, civil and citizenship rights of people living with HIV infection (PLWHA) and all vulnerable groups (young people, women, sex workers, drug abusers, inmates, migrants) also by building human and social integration services.

    Specific aims of LHIVE are to:

    • defend the rights of PLWHA and vulernable and marginalised people
    • bring out their needs and make them visible, try to provide the first aid and stimulate institutions to take charge of them
    • develop and manage pilot experiences to defend the rights of PLWHA and vulnerable people
    • plan and manage pilot experiences for the prevention of sexually transmitted infections among vulnerable people and in the general population
    • plan and manage pilot experiences to improve adherence to therapist of PLWHA
    • reduce the spread of sexually transmitted infections
    • promote physical and mental health in fragile groups

    Within the Association there are the following work groups:

    • Group of HIV, sexually transmitted and blood-borne infections
    • Group on development of projects and training courses
    • “Group Africa” to coordinate support activities for migrants living in Sicily and activities to fight AIDS in Zimbabwe
    • “Gruppo Antartide” to address issues relating to drug use and drug abuse

    Voluntary services carried out with own funds:

    • Reception service: on-site meetings with LHIVE operators
    • Telephone help line: consultancy and territorial observation activities
    • Medical-psychological counseling: individual, couple or family interviews at the headquarters
    • Self-help groups for PLWHA
    • Salivary test for the diagnosis of HIV infection and Hepatitis C
    • Legal advices
    • Social Secretariat to provide assistance aimed at obtaining disability pension
    • Information/training activities, addressed to young people, teachers, students, nurses, psychologists, social workers, physicians, educators, migrants, sex workers, drug users, inmates
    • Internal training (members, volunteers, LHIVE operators)

    Activities carried out and/or supported in Zimbabwe with own funds:

    • Progetto “Susy Costanzo” – Diamo vite all’Africa. The project aims to raise funds to support the activities of Luisa Guidotti hospital based in Mutoko, Zimbabwe, in particular to provide free services to destitute people living in villages. The project also provides for the periodic dispatch of health personnel. The project started in 2004 and is still on going.

Ireland

Healthcare rights of migrants

Asylum seekers: Access to healthcare is free for protection applicants living in Direct Provision. Asylum seekers must apply for a medical card which allows them to attend local doctors or general practitioners located in the Direct Provision accommodation centres. This card entitles them to prescribed drugs and medicines and their status as applicants living in Direct Provision makes them exempt from paying prescription charges levied on medical card holders. Specialised treatment for trauma and victims of torture is available through an NGO called SPIRASI.

Refugees: Refugees receive the same medical services that they did as asylum seekers as there is no difference in the provision for asylum seekers and people with refugee status, subsidiary protection or permission to remain.

Undocumented migrants: Undocumented migrants can access emergency care for a fee. 

HIV testing and treatment

Testing: Free access for all in clinics and NGOs only. Hospital consultants and GPSs in private practice charge a fee. Use the European test finder to see where you can get tested for HIV. 

Treatment:  Free for all. However, clinicians decide who and when to treat. Undocumented people may sometimes be excluded.

Healthcare rights of migrants

Asylum seekers: Upon arrival asylum seekers undergo free medical examinations to see if they have any contagious diseases that require treatment. They are also entitled to free perinatal and postnatal care, as well as mental health treatment from social workers, psychiatrists and psychologists. Asylum seekers who obtain a work permit have the same requirements as legal migrants, which means that they have to purchase health insurance during the first six months of residence, after which they can become insured by the national health insurance. For imprisoned asylum seekers, they have the right to emergency health care in cases of sudden illness or accident.

Refugees: Upon arrival in Iceland, all refugees undergo a medical check-up. Medical and dental care is provided as required, free of charge, during the first six months of residence in Iceland. After six months, refugees have the same access to the health system as Icelandic citizens. Psychological assistance is provided for a longer time free of charge.

Undocumented migrants: Are not covered by the regular state health insurance system. Nevertheless, they are entitled to receive emergency health care in case of sudden illness or accident. They are expected to pay for the emergency care provided, but if they are unable to do so, the cost will fall on the service provider. Undocumented migrants cannot purchase private health insurance.

Hungary

Healthcare rights of migrants

Asylum seekers: Access to healthcare is provided for asylum seekers as part of the reception conditions. It covers essential medical services and other free medical services provided to legally residing third country nationals. Asylum seekers have a right to examinations and treatment by general practitioners, but all specialised treatment conducted in polyclinics and hospitals is free only in case of emergency and upon referral by a general practitioner. Asylum seekers have access to a general physician in the town of Vámosszabadi several times per week and to nurses daily and there is an Arabic speaking social worker who assists with the translation.

Refugees: Have access to the national health system, which is covered by the Social Security scheme, under the same conditions as Hungarian citizens.

Undocumented migrants: Can access emergency care free of charge.

HIV testing and treatment

    The idea of Society Kitchen “The Other Human” Free Food For All is an action of solidarity where people come together to make food for the afflicted. Run and funded by volunteers, including people who are unemployed or homeless, the food is prepared in a public space, and while they wait for it to be cooked, they communicate with one another. The initiative also holds weekly classes for children who are dropping out of school, and runs a theatre group that provides art therapy.

Our clients face many obstacles in accessing, and violations of, their rights. On Lesvos, we offer essential legal information and counsel inside the refugee camp, assisting over 1300 people this year. In Athens, our attorneys offer full legal representation throughout the entire asylum process.

    Job counselling, job search services on the internet with the help of specialised counsellors, group workshops, seminars and networking activities (seeking to enhance the ability of the unemployed by vulnerable groups to enter the labour market).

  • Greek Forum of Migrants

    The Hellenic Immigrant Forum (EFM) is a network of 42 immigrant organisations and communities in Greece for the protection of immigrants’ rights and equal employment and social integration.

  • Red Umbrella Athens

    Provides free prevention and empowerment services to women (trans & cis) and male sex workers at the Day Care Center.

  • METAdrasi

    Facilitates the reception and integration of refugees and migrants in Greece, by providing services such as interpretation, accommodation for unaccompanied children or legal aid to asylum seekers.

  • Syrian American Medical Society Organisation

    Provides medical care and treatment to every patient in need.

  • GIVMED

    Support the elderly, refugees and other socially vulnerable groups who do not have access to their medicines.

  • I Have Rights

    I Have Rights (IHR) is a non-profit legal and political organisation providing access to individualised legal information and support to people on the move, including people seeking asylum, refugees, and migrants on Samos, Greece. We communicate openly about the situation on Samos while also advocating for the change of policies and practices that are responsible for human rights violations on Samos and in Europe. Our organisation addresses present-day unfair and undignified asylum and migration procedures by working to ensure the right to asylum, eliminate pushbacks, and fight against all detention of migrants and refugees.

Germany

Healthcare rights of migrants

Asylum seekers: The law restricts healthcare for asylum seekers to instances of acute diseases or pain, for which necessary medical or dental treatment has to be provided.

Refugees: Refugees are not entitled to the German statutory health insurance scheme, but are entitled to medical treatment for acute illnesses, medical care for pregnant women and those who have recently given birth, basic dental care, preventive vaccinations and other essential medical services for a particular illness. After spending more than 15 months in Germany, they can enjoy the same benefits as German citizens.

Undocumented migrants:  Undocumented migrants have access to emergency care and several categories of planned care for those who possess a medical card. It is important to know however that to obtain a medical card, undocumented migrants must apply at the welfare office. The welfare office must report the individual to the authorities, which may lead to deportation.

HIV testing and treatment

Testing: Free in certain places. Some public health centres may charge fees due to health insurance needing a name to pay for the testing fees. Testing is anonymous if you go to an NGO. However, if getting tested at a clinic, testing is only anonymous for a certain fee. Use the European test finder to see where you can get tested for HIV.

Treatment:  Free for documented citizens. Treatment is reimbursed by health insurance. Those without health insurance tend to be excluded.

    Lobby organisation for political and civil society actors that supports migrants and their families, and aims to promote intercultural integration.

  • Fixpunkt e.V.

    Fixpunkt eV is the sponsor of health promotion and addiction aid projects with a focus on “infection prophylaxis”. Areas of work are the operation of syringe machines, low-threshold mobile social work and medical support to reduce harm and infection prophylaxis in the context of drug consumption, medically supervised drug consumption, dental prophylaxis and the promotion of self-organization by drug users.

Healthcare rights of migrants

Asylum seekers: Are entitled to free health insurance while their application is being reviewed. Persons who have no right to remain in the territory, including rejected asylum seekers, benefit from the PUMA for six months after the end of validity of the asylum claim certification. After this period, State Medical Aid (AME) enables them to receive free treatments in hospitals as well as in any doctors’ office. Individuals with low income who are still awaiting health insurance and need healthcare quickly can go to the All-Day Healthcare Centres (PASS) at their nearest public hospital.

Refugees: Have the same rights to access the medical system as French citizens and legal residents.

Undocumented migrants:  Due to universal healthcare, undocumented migrants can access the same healthcare benefits as French nationals through the State Medical Assistance (AME), including emergency care, paediatric care, dental care and maternity care.

Treatment:  Free for all people who have been in France for more than three months. Undocumented migrants also need to have an income of less than 700 euros per month.

Finland

Healthcare rights of migrants

Asylum seekers: Reception centres are responsible for arranging healthcare services for asylum seekers. Their entitlement to reception services ceases if their application for asylum is rejected or their temporary protection ends. Minor asylum seekers are entitled to the same healthcare services as locals. Specifically, children under 7 years receive health and development screening at child health clinics and all children are vaccinated. Additionally, vulnerable people (elderly, pregnant women, patients with disability, long-term illnesses or traumatised people) receive all necessary health services.

Refugees: Refugees who have been granted a residence permit are covered by the Finnish health insurance and are granted a Kela card, which entitles them to receive Kela reimbursements for private health care, medication and travel costs. Once a place of residence has been granted, the refugee has the right to use all public healthcare services in the same manner as other Finnish residents and for the same client fee as local residents.

Undocumented migrants:  Can only access emergency care and are required to pay for it.

Healthcare rights of migrants

Asylum seekers: Can receive basic primary services and emergency hospital services. Asylum seekers required to apply for specialised treatment for life threatening and painful chronic diseases, which are financed by the Danish Immigration Service.

Refugees: Are entitled to free access to healthcare if they have been registered as residents for more than six weeks. Children under the age of 16 are covered by the same form of coverage as their parents. 

Undocumented migrants:  Can access emergency care free of charge, which is covered by the Danish Immigration Service. They may in some circumstances access primary and secondary care in return for payment of the full cost.  They may enquire at one of the asylum centres or at the Danish Immigration Service when in need of treatment.

    Legal advice and political group working for LGBT people.

  • LGBT+ Denmark

    Works against the discrimination and provides legal advice to LGBT+ people in areas such as marriage, adoption, the artificial insemination of lesbians, and rights for trans persons.

Asylum seekers: Are entitled to basic health and mental healthcare.

    Provides social and health services to people in Czech Republic. This includes medical care, and the distribution of food, water and clothing.

Cyprus

Healthcare rights of migrants

Asylum seekers: Are entitled to free public healthcare if they do not have the means to support themselves. They have to apply for a medical card at the Ministry of Health or the General Hospital in their district in order to have access to the public healthcare system. Note that asylum seekers are not covered by the General Health System, and thus cannot access personal GPs.

Refugees: Are entitled to healthcare under the same conditions as Cypriot nationals. Healthcare is received under the General Health System (GeSY). 

Undocumented migrants: Can access emergency care free of charge. They may also access primary and secondary care in return for payment of the full cost.

HIV testing and treatment

  • Generation for Change CY (GFC)

    Generation for Change CY (GFC) is a migrant-led grassroots non-profit organization comprised of individuals from diverse backgrounds, united in their mission to foster a diverse and inclusive society where everyone has equitable access to opportunities in Cyprus. GFC is dedicated to driving positive social change by supporting, empowering, and advocating for vulnerable and marginalized communities, including but not limited to individuals of low socioeconomic status, refugees, asylum seekers, migrants, and anyone facing discrimination.

    GFC provides material assistance to disadvantaged individuals and families and implements projects to integrate and empower these groups. These initiatives include language learning, vocational education and training, psychosocial and well-being support, and sports programs for both adults and children. Additionally, GFC organizes advocacy campaigns, social initiatives, and cultural projects to raise awareness of social issues and promote intercultural exchange.

  • AIDS Solidarity Movement

    Provides support for people living with HIV and their partners, friends and anyone they consider family. The Movement’s CBVCT Centre, “Cy Checkpoint”, offers anonymous, free and rapid HIV/Syphilis testing, counseling, information and linkage to care to Migrants, refugees, asylum seekers, LGBTI+ people, sex workers and the general population.

  • Caritas Cyprus

    Provides support and information for migrants in need to access their legal rights and basic needs.

Healthcare rights of migrants

Asylum seekers:

Can only access emergency medical assistance and necessary treatment of illness through Doctors of the World (in cooperation with national and local health institutions). Doctors of the World also provides mental health support and support to health referrals in reception centers for asylum seekers. Prenatal and postnatal care are not included under emergency. Children under 18 can access the same healthcare rights as nationals. 

Refugees: Are entitled to comprehensive healthcare and have the same healthcare rights as Croatian citizens.

Undocumented migrants: Can access only emergency healthcare.

HIV testing and treatment

Testing: Testing is free, anonymous, and available in 10 locations across the country. Use the European test finder to see where you can get tested for HIV.

Treatment: Treatment is free for those who have health insurance.

Healthcare rights of migrants

Asylum seekers:

Are entitled to the same healthcare rights as nationals. Health insurance is covered by the SAR.

Refugees: Are entitled to the same healthcare rights as nationals. However, upon receiving refugee status, refugees that are uninsured have to pay for their own medication.

Undocumented migrants: Can access emergency care in return for payment of the full cost. 

HIV testing and treatment

Testing: Testing is free and anonymous testing for all, when done at the Regional Health Inspectorates in district cities and in the mobile units (NGOs). Use the European test finder to see where you can get tested for HIV.

Treatment: Treatment is covered, except for undocumented persons.

    Single Step’s mission is to support, motivate and empower LGBTI (“Lesbian, Gay, Bisexual, Trans and Intersex”) youth, their families, friends and allies in Bulgaria through the process of recognizing, coming out and affirming their sexual orientation and gender identity.

    Single Step provides accessible, affordable, supportive, and non-judgmental mental health and social services for LGBTI emerging adults in Bulgaria:
    1) A free, anonymous online chat staffed by trained volunteers, offering emotional support and crisis intervention;
    2) Individual psychological consultations and support groups for LGBTI youth, their parents, friends and allies;
    3) A network of trained psychologists in 10 cities around the country;
    4) A career program offering career advice and educational opportunities to emerging LGBTI youth;
    5) A support program for transgender and non-binary individuals, including hormone therapy information, surgical options, and legal advice (partnering with a local LGBTI organization);
    6) A health programme offering consultations and resources on sexual health, HIV and STI prevention;
    7) The Steps, a vibrant community center and social enterprise that offers a safe and inclusive space for LGBTI individuals to socialize, network, and participate in cultural, entertainment and other community events.

  • Foundation for Access to Rights (FAR)

    The Foundation for Access to Rights – FAR is a Bulgarian non-governmental organization in the public interest, founded by attorney-at-law Valeria Ilareva in 2013. FAR laywers provide free legal aid to asylum seekers, refugees and migrants in five regions in Bulgaria: Dobrich, Varna, Bourgas, Plovdiv and Sofia. Apart from the field offices, FAR provides online information and legal consultations via its platform www.refugeelight.bg. After more than 10 years of practical experience in providing legal aid to foreigners in Bulgaria, in 2023 the Foundation for Аccess to Рights – FAR created the job-matching migrantlife.bg platform as a response to the demand of employers to find workers among the migrant communities in Bulgaria, as well as the need of foreigners to be included in society and to be economically independent. FAR is a licensed labor mediator under № 3570 from 01.08.2023 under the Ministry of Labour and Social Policy of the Republic of Bulgaria.

Healthcare rights of migrants

Asylum seekers: Are entitled to primary health care. For advanced services, a minimal fee must be paid. Healthcare is covered either by Fedasil or the local OCMW/CPAS where asylum seekers reside. Asylum seekers are exempt from franchise patient fees, unless they receive financial allowance or earn a professional income. Rejected asylum seekers can have their healthcare costs covered by their reception centers. For those that are no longer entitled to reception, urgent medical assistance is still accessible and covered by their local OCMW/CPAS.

Refugees: After registering through the Belgian health insurance organization, refugees can access healthcare at the same level as Belgian nationals.

Undocumented migrants: Cannot affiliate with a sickness fund. However, they can contact the Public Centre for Social Action (CPAS) of their municipality to access healthcare via a procedure called Urgent Medical Aid (UMA). The UMA in principle covers preventive healthcare as well as treatment and therapy for illnesses that are considered curable. 

HIV testing and treatment

Testing: Testing is free and anonymous at 3 sexual health centers (Antwerp, Brussels and Liege) and at community-based organisations. Use the European test finder to see where you can get tested for HIV. 

Treatment: Treatment is free for some. Undocumented persons may be refused medication in some cases. For more information on where to obtain treatment, see hivtravel.org.
